ARDMS abdomen GI Tract + abdominal wall[국제초음파자격증]

이제 몇 chapter 안남았는데 최선을 다해서 꼭 마치도록 하겠습니다!!

 

Gut layer(5layer)

superficial mucosa--------------------fine bright line

deep mucosa--------------------------gray

submucosa-----------------------------bright

muscularis propria---------------------dark

serosa-----------------------------------fine bright line

 

gut wall 평균 두께는 약 3~5mm

gut의 비정상적인 모양 : target / asymmetric target 모양

accessary gland of the GI Tract : 침샘, 이자, 간

pathology

1. acute appendicitis(급성충수염) [진단에 도움 되는 곳 : terminal ileum]

(1) 위치

RLQ + Cecum

(2) probe 주파수

5.0 MHz linear with short focus

(3) mcburney point

halfway between the anterior superior iliac spine and the umblicus(압을 제거할 때 통증)

(4) 눌렀을 때 압박되지 않는다.(noncompressible)

(5) 염증이기 때문에 백혈구 증가, doppler 증가(hypermic flow)

(6) appendicolith 존재 가능 + 충수주위 fluid 존재 가능

(7) wall 두께 : 6mm 이상(measure : outer wall ~ outer wall)

(8) 급성 충수염을 알 수 있는 혈액검사는 존재하지 않는다.

 

2. HPS (유문 협착증)

(1) 발생시기 : 유아(2~6주), 성인

(2) abnormal muscle의 두께 : 3mm

    abnormal muscle의 길이 : 17mm

(3) palpable olive sign :  an enlarged palpable pyloric sphincter

(4) 관련있는 단어 :  midgut malrotation[SMA와 SMV 위치가 바뀜]

                          Pylorsoasm, gastroesphageal reflux disease

 

3. intussuception(장중첩)--> 급성질병

(1) 증상

: red currant jelly stools, vomiting, leukocytosis, noncompressible, target shaped or pseudokidney shaped mass

(2) 치료

: enema

 

4. 장폐색 intestinal obstruction = ileus

(1) 2살 이하 어린이에게 가장 잘 발생하는 원인 : 장중첩증

(2) 초음파적 특징

: distended fluid-filled loops of bowel

 

5. crohn disease

: 궤양성 대장염, 전체적인 염증, terminal ileum and colon 침범 

(1) 정의

autoimmune disorder characterized by period of inflammation of the GI Tract.

(2) 임상적 증상

설사, 복부통증, 체중감소, 직장출혈

(3) 초음파적 특징

bowel wall thickening, noncompressible, hypermic wall

 

abdominal wall pathology

1. rectus sheath hematoma

(1) 혈종이기 때문에 hematocrit 감소

(2) Rupture of epigastric vessel

(3) 항응고제 치료를 받았을 때 나타날 수 있다.

 

2. abdominal hernia

(1) 탈장 :  신체의 장기 또는 조직이 그 주위를 둘러싼 병의 결손을 통해 비정상적으로 돌출된 상태

(2) 도움이 되는 기법 : valsalva
